Related Service Logs for IEP Students

Per Illinois law, the following serves as a notice for parents/guardians of students with disabilities who may receive related services as part of their Individualized Education Plan (IEP)

Triad Community Unit School District #2 will maintain related service logs that record the delivery and number of minutes of the related service/s administered to such students. Parents/Guardians of students with disabilities may request copies of their child’s related service logs at any time.
